Step-by-Step Process to Become a Vendor

1. Watch this training and orientation presentation for all new vendors.
2. Review the Vendor Code of Ethics.
3. Familiarize yourself: DVR Fee Schedule in PDF or DVR Fee Schedule in Word - October 2023.
4. Submit all documentation via the DVR Registration Form. It's important to connect with your local office to understand their procedures and the staff you'll be working with.
5. Next steps: The Quality Services team will contact you for additional information and updates to your application status.
